The Transition and Post Care Support (TPCS) Officer provides direct support to young people with a care experience aged 15-21 years with complex support needs (e.g. disability, significant mental health concerns and/or high risk behaviours) who are most at risk of homelessness as they transition to adulthood and post-care. Your work will focus on the development of protective independent living skills, family and support networks and access to necessary specialist supports relevant to the young person's risks and needs.
The TPCS Officer works in collaboration with Child Safety staff, government, and non-government agencies to improve young people's quality of life, achieve stability in their living situation, and increase their involvement in schooling, employment, their family, culture and community. This role supports the Southeast region inclusive of Bayside, Beaudesert, Beenleigh
• Drivers Licence - The possession an Australian "O" (open) or "P2" (provisional green P plate) and "C" or "CA" class driver's licence.
• Ability to undertake routine intrastate travel for short periods.
• A positive Blue Card or ability to obtain a positive Blue Card prior to commencing in this role.
• Experience in working with high-risk young people with complex support needs It is highly desirable that employees are vaccinated against COVID-19.
• NOTE: Employees unable to comply with any future mandatory vaccination policy requirements of the Department may be subject to disciplinary action.
Key duties and responsibilities The Transition and Post Care Support Officer key responsibilities are:
• Provide specialist transition from care support that identifies and appropriately responds to the complex support needs of high-risk young people who are at risk of homelessness based on the principles of the Strengthening Families Protecting Children Framework for Practice and the core elements of the Aboriginal and Torres Strait Islander Child Placement Principle.
• Promote and facilitate involvement of the young people in goal setting and planning for their transition while supporting their connection to family, culture and community.
• Organise the provision of specialist services that address high risk and complex needs of young people and assist with the development of young people's independent living skills with a central focus on enhancing capacity to maintain safe and stable housing aid their wellbeing and post-care outcomes and reducing their risk of homelessness.
• Consult and develop relationships with government and non-government agencies including Queensland Health, Youth Justice, Office of the Public Guardian, Department of Housing and Public Works, and the National Disability Insurance Agency to ensure efficient, effective and financially supported service delivery.
• Collaborate with regional line manager and senior program officer within a hub and spoke model ensuring continuous quality improvement and alignment with regional and programmatic priorities and identify opportunities for outcome improvement when supporting young people transitioning from care.
• Use specialist transition practice knowledge to contribute to improvements in service delivery and enhance workforce capacity in child safety and external agencies.
• Maintain sound, accountable and legislatively compliant record keeping relevant to service delivery including contribution to programmatic data reporting systems.
• Prepare communication materials, professional development, and other relevant documentation relevant to the service, including letters, reports, meeting minutes, PowerPoint presentations and associated written communications.
