The role The focus and purpose of this role is to provide excellent case management support to children and young people, families, significant others and relevant stakeholders within our Intensive Therapeutic Care Program.
The children and young people you support may have a disability or have experienced complex trauma backgrounds, and as such have complex and/or challenging needs.
The Therapeutic Case Manager will enable our children and young people to improve their quality of life and participation in the community, ensuring that service delivery is person-centered and based on a therapeutic framework focusing on strengths-based intervention and support.
About you If you're able to work collaboratively in a multidisciplinary team and have experience working with children and young people who have experienced complex trauma backgrounds, we'd love to chat with you!
Tertiary qualifications in Social Work, Psychology, Nursing & Mental Health (or have completed the first 12 months) Experience with Case Management in the Community/Social sector Excellent verbal and written communication with the ability to write clear, detailed case notes Knowledge and demonstrated experience to work within the relevant legislation, OOHC Standards, policies, and procedures. Capacity to apply culturally appropriate case management practices with Aboriginal and culturally diverse communities Driver's license Working with Children Check Ability to work flexible hours We offer Hourly rate based on SCHADS Award Salary packaging $15,900 general and $2,650 entertainment Paid training and opportunities for career development 24/7 crisis and incident response support Free confidential counselling support for employees and close family members About us We provide services to people with disabilities, young people, and children in out-of-home care, their families, and communities.
