About Louise Place   Louise Place provides vital support and accommodation to young women aged 18 years and under who are pregnant and/or parenting and facing homelessness or the risk of homelessness.
The service offers therapeutic case management for young women and their children, both in the Louise Place Core and Cluster units and through our outreach program.
The Role   This is a full-time fixed-term role until 30 June 2025, based in Fullarton within our Domestic Violence, Homelessness and Youth Services unit.
As the Team Leader at Louise Place, you will oversee case management services for young parents and their children, supporting them through complex challenges including independent living, mental health, substance misuse, parenting, and housing.
You will play a critical role in developing tailored case plans for both parents and their children, with a strong focus on addressing the impact of homelessness on children's health, attachment, and development.
In this position, you will report to the Senior Manager and collaborate with other site Team Leaders, social workers, and support staff to ensure high-quality care and service delivery, following the approved Case Management Guidelines and the Young Family Support Service Model.

The key duties in this role will include:   Oversee the roles of the Family Support Worker and Children's Worker, providing direct supervision, support, and managing rostering.
Lead and implement quality practices within the service, focusing on trauma-responsive care, life span development, attachment, grief and loss theories, and a strengths-based approach.
Build and maintain strong working relationships with both government and non-government stakeholders to support collaboration.
Actively participate in and promote multi-agency case conferencing to ensure the best outcomes for clients.
Coordinate referrals and liaise with relevant agencies to ensure integrated support and optimal client outcomes.
Provide timely feedback to the Senior Manager to enhance and improve service delivery responsiveness.
About You   You are a compassionate and experienced professional with a deep understanding of the challenges faced by young people experiencing homelessness and related issues.
Your extensive knowledge of intergenerational trauma, abuse, and neglect allows you to approach these complexities with empathy and insight.
With excellent interpersonal and communication skills, you excel at building trust with young parents, empowering them to share their experiences and guiding them toward independent living.
Your natural leadership abilities ensure that team harmony is maintained, fostering a supportive environment where each team member feels valued and assisted as needed.
You are committed to creating a positive impact in the lives of those you serve while nurturing a collaborative and effective team dynamic.
Essential Criteria   Social Work Degree Ability to achieve effective client outcomes through quality case management Experience working with vulnerable young people with complex needs and an understanding of the impact of childhood trauma, attachment theory, child and adolescent development Knowledge of the homelessness sector and intersecting services and agencies Proficiency in computer use and ability to learn new technology Written communication skills including preparation of concise and factual case-notes and progress/closure reports, and DCP Section 152 Reports.
