About You

We're looking for someone with the following attributes:
The ability to effectively and quickly build rapport with people
Effective listening skills, an adaptable communication style and a desire to learn more
Contribute positively within a team and work autonomously
Familiar with technical terminology in a clinical setting
Previous experience in mental health or outreach support (professional, volunteer, lived experience), supporting people with complex needs; ABI / AOD / mental illness / physical disability / dual diagnosis.
Qualifications in Mental Health, Social Science or other relevant qualification (or be actively studying) is beneficial
It is a requirement of this position to meet all compliance requirements in relation to NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver's licence.
About the Role
As part of our TAC team, you will support participants on WorkCover and TAC throughout their recovery journeys. Our participants may have a mental health condition, Acquired Brain Injury (ABI), or disability resulting from a workplace or traffic incident. Your role will be to help them build the skills and confidence to manage daily tasks independently, with a strong focus on empowerment over direct intervention.
The role operates in a hybrid model, combining work-from-home, office (Bendigo), and outreach support.
You'll need a personal vehicle for work-related travel within a 110 km radius of Bendigo
(You will be reimbursed for your travel expenses as appropriate).
This role entails:
Community outreach and telehealth support, case management and following established behavioural support plans
Regular communication with care teams, linking participants to a wide range of community and clinical services
Developing professional relationships with individuals, their families and other stakeholders in the community
Managing referrals, performing risk assessments, providing accurate and timely case notes and reports
Use of a variety of systems and navigate multiple platforms
This is a part-time role (3-4 days per week) with work hours from 9:00 am to 5:00 pm, flexible between Monday to Friday. Tuesdays are mandatory workdays.
