About the Role: Support CoordinationBased out of our Moonee Ponds Hub.Part Time Position - 4 days per weekFixed Term 6-month contract.Lifestyle Solutions provide innovative and flexible person-centred support coordination services for people living with a disability. Support Coordinators assist our diverse clients to become more independent in their own homes and in the community.
Your responsibilities will include:
Actively coordinate, monitor and report on supports in accordance with clients' plansActively support clients to meet their individual goals by identifying and accessing necessary services & supportsDemonstrated knowledge and experience of the NDIS, including legislation, regulations, policies, and practicesPrior experience in a similar Support Coordinator positionStrong interpersonal and stakeholder engagement abilitiesProven ability to achieve KPIsProven ability to meet administration, report writing and compliance obligationsExcellent time management and prioritisation skillsStrong written and verbal communication skillsProficiency in Microsoft Word, Excel, Outlook, Teams, with intermediate - advanced computer literacySupport effective communication and partnering with identified stakeholders (i.e.: service providers, mainstream services, funding agencies, government departments)About You: Current Driver's License (mandatory requirement)NDIS Worker Screening Clearance (or be willing to obtain)Formal qualification in Social Sciences, Disability, Community Services, Psychology, Allied Health and/or equivalent experience (minimum Cert IV)Benefits: SCHADS Award Level 4Team based structure and an inclusive culture - you will have autonomy and the opportunity to make decisionsSalary sacrificing optionsOngoing paid training and development (internally and externally)About Us: We're people who care.
