Support Coordinator

This position is located in Far North Queensland (FNQ), Cairns, Australia. As a Support Coordinator and a key member of our growing and dynamic team, you will assist participants in the coordination and implementation of all supports through their NDIS funded plans.
You will regularly communicate with participants, their families and carers to best maximize the quality of support and value for money and build on your own knowledge for the best available services. You will also work as part of a team of Support Coordinators and be under the direction of a Team Leader.
You will be accountable for:
Completing support coordination within an agreed time frame; Managing a portfolio of participants based on person-centred plans including ongoing tracking of billable hours; Proactively taking part in participant review practices and coordinating a range of supports including informal, mainstream and funded; Build high-quality professional relationships with internal and external stakeholders; Develop, maintain and complete relevant documentation including service agreements; Capture relevant data required by the NDIA; Preparation of NDIA reports on outcomes, success indicators and financial tracking. #J-18808-Ljbffr
