Position Overview: We are looking for an experienced NDIS Support Coordinator to join our dynamic team in January 2025. The successful candidate will be responsible for providing support coordination services to NDIS participants, including Level 1, Level 2 & Specialist Level 3, assisting them in navigating and accessing the support services and resources they need to achieve their goals and live independently.
Responsibilities: Provide support coordination services to NDIS participants, including conducting assessments, developing support plans, and coordinating services.Work closely with NDIS participants to identify their goals, preferences, and needs, and develop individualized support plans to help them achieve their objectives.Liaise with service providers, community organizations, and other stakeholders to coordinate and arrange support services, ensuring that participants receive the appropriate assistance and resources.Monitor participants' progress and review support plans as needed, making adjustments to ensure that services are meeting their evolving needs and goals.Advocate on behalf of participants to ensure they have access to the services and supports they require to maximize their independence and quality of life.Maintain accurate records and documentation in accordance with NDIS guidelines and organizational policies.Keep up-to-date with changes to NDIS policies, procedures, and guidelines, and ensure compliance with regulatory requirements.Requirements: Bachelor's degree in social work, psychology, disability studies, or a related field highly regarded.Minimum of 2 years of experience working in disability support services, with specific experience in NDIS support coordination.Strong understanding of the NDIS framework, including knowledge of NDIS legislation, policies, and procedures.Excellent communication and interpersonal skills, with the ability to build rapport and establish positive relationships with NDIS participants, families, carers, and service providers.Ability to conduct assessments, develop support plans, and coordinate services in accordance with participants' individual needs and goals.Well-developed organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities effectively.Compassionate, empathetic, and client-focused approach to service delivery.Ability to work autonomously and as part of a multidisciplinary team.Current driver's license and access to a reliable vehicle.Valid Working with Children Check and National Police Clearance (or willingness to obtain).Benefits: Competitive salary based on qualifications and experience.Opportunity to make a meaningful difference in the lives of individuals with disabilities.Supportive and collaborative work environment.Ongoing professional development and training opportunities.Flexible work arrangements (Work from Home)If you are a passionate and dedicated professional with experience in NDIS support coordination, we encourage you to apply for this rewarding opportunity.
