We are a Brisbane based team, supporting NDIS participants across South-East Queensland both face-to-face and remotely.

Key Duties Strive to represent Hively Coordination in a person-centred, respectful and professional manner at all times.
Partner with participants and their families to provide person-centred Support Coordination services for NDIS participants, including:Support of participants to understand and implement their NDIS plan and to build capacity and independence Review of support arrangements to understand each individual client and to provide an ongoing person-centred care planDetermine and work within the funding level of each participant, including support of participants to access further funding if requiredSupport and management of plan review processes, including unscheduled reviews and internal complaints processes to ensure quality outcomesAdvance planning for potential crisis management and advocating for change to promote client safety and wellbeingDelivery of timely person-centred supports Facilitate effective working relationships and open communication with participants, carers, family, external services and other key stakeholders (including but not limited to community organisations, NDIS and non-NDIS service providers, government organisations)Develop and maintain strong relationships across the sector to ensure participants choice and controlUndertake tasks as directed by the Management team, including attending all staff meetings and training opportunitiesCommitment to best practice and person-centred outcomes across the sectorUphold and drive a culture of collaboration, respect, and continuous improvementYou can expect from Hively Coordination Flexibility – full-time and part-time employment available with the ability to work flexibly from homeExcellent pay and conditions (pay commensurate with experience)Industry standard KPI's Supply of company IT (laptop, phone)Travel allowanceSupportive work environment – support from management, team leader and our friendly support coordinators Professional development – ongoing training and career development / progression opportunitiesA team of amazing professionals committed to making a difference!!Required Previous experience in a similar position or ability to demonstrate transferable skillsAbility to work within a person-centred framework Demonstrated ability to manage competing priorities and deliver quality outcomesWell-developed interpersonal and communication skills, both written and verbalExcellent organisational and time-management skills Ability or proven ability to meet KPI'sExceptional written and verbal communication skillsExceptional interpersonal skills, including the ability to connect with participants in a professional and empathetic mannerExperience of or demonstrated ability to work in support planning and case managementDemonstrated ability to network and develop sound relationships with local service providersDesired Tertiary qualifications desirable but not essential: Psychology, Social Work, Occupational Therapy or other Allied Health disciplinesCertificate IV in Disability, mental health or other qualification relevant to the field of human services with relevant experience.Knowledge of or the ability to rapidly acquire knowledge of the Disability Services Act, National Disability Insurance Scheme Act, National Disability Services Operational Guidelines and the NDIS Operational GuidelinesLived or learned experience with disability What we need from you Current Class C Driver's LicenseReliable access to a roadworthy vehicle with current registration and insuranceQueensland Working with Children Check (Blue Card) NDIS Disability Worker Screening Check 100 Points of ID Valid visa We want to hear from you!!
