- Working across 7 days Join the team as a Social Worker and help support our commitment to quality patient care!We are seeking enthusiastic compassionate Social Workers to join our Mental Health Hospital in the Home Team!Mental Health Hospital in the Home (HITH) is an alternative option to hospital admission for the person experiencing an acute episode of mental illness.
The aim of HITH is to provide high quality evidenced based interventions, treatments, and therapies in the person's own home; enabling them to feel valued, empowered and connected to family or carers.
About the Role:Reporting to the Nurse Unit Manager and the Principal Social Worker, the HITH Social Worker will also undertake clinical care as part of the multi-disciplinary team within the HITH.
Responsibilities: Collaborate with members of the multi-disciplinary team, consumers, carers, community health teams and other community agencies in the development and implementation of treatment plans.Undertakes a range of recovery orientated services, including mental health and psychosocial assessments, therapy and educative programs to consumers, family and carers during the period of admission to HITH.Apply specialised expertise to the development, delivery and continuous improvement of quality clinical work and provides discipline specific assessments and interventions.
Assist with the development and establishment of key performance indicators for all critical activities relevant to area of responsibility in accordance with the quality evaluation program.About you: An appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.Ability to provide core social work skills including psychosocial assessment, counselling, individual and group work, relationship and family interventions, crisis intervention and brief therapy.Ability to undertake comprehensive assessment skills including, consumer needs analysis, assessment skills in mental state and risk assessment.Ability to work collaboratively with individuals, families, carers, other agencies and community services to implement Treatment and Care Plans and coordinate comprehensive service provision for consumers who have enduring and complex needs.Ability to understand the value systems and cultural differences of people from diverse backgrounds.
