Your opportunity
To deliver high level comprehensive Social Worker clinical skills and leadership to provide holistic, coordinated care to patients and their families engaged with the Connected Community Pathways Service (CCP) multi-disciplinary team.This position reports operationally to the Director of Nursing Facility Manager (Bowen) and professionally to the Director of Social Work (MHHS).About the Team
Bowen is situated at the top of the Whitsundays about 2 hrs drive from two larger towns, Mackay to the south, and Townsville to the north. Bowen Hospital is a 27 bed Hospital, comprising sub-acute inpatient beds, emergency department, and oncology and renal day patient services. The Bowen community supports two Aged Care Facilities and multiple home care support services.
Funded via the Connected Community Pathways (CCP) project, this Social Work position sits within a multidisciplinary team, which is led by a Frail Aged Nurse Practitioner. This service specifically focuses on improving the health and wellbeing of older people aged 55 years and above who reside in Bowen, including reducing failed hospital discharges and re-admission. The role is diverse and interesting and covers inpatient and outpatient services.
