This role involves providing comprehensive psychosocial assessments, therapeutic counselling, case management, and individualised therapeutic interventions to enhance the mental health and wellbeing of older adults living in residential aged care facilities. Additionally, you will work closely with families and carers to provide guidance and support, ensuring they feel empowered to contribute effectively to the wellbeing of their loved ones.Your role will also focus on supporting residents and their families to make meaning of life experiences, with an emphasis on strengths and values. You'll have the opportunity to broaden your expertise in areas such as grief and loss, anxiety, depression, trauma, life transitions and adjustments, health diagnosis and loss of independence, as well as palliative and end-of-life care. This is a casual role with a minimum of one day per week, based in the Devonport area.About YouSkills and Experience:Social Work Experience: Demonstrated experience working within the mental health, aged care, palliative care, and/or health sectors.Client-Centred Approach: Passionate about supporting older adults to live with dignity and improving their mental health and wellbeing during challenging times.Counselling Skills: Proven experience providing therapeutic counselling using evidence-based modalities tailored to older adults.Clinical Skillset: Strong clinical knowledge and skills for managing complex psychosocial issues, completing comprehensive assessments, and navigating services and organisations relevant to older adults.Advocacy and Case Coordination: Proven ability to advocate for the needs and rights of older adults within the health and social care system and to coordinate care effectively.Independent Practice: Demonstrated ability to work autonomously and confidence in practising as the sole social worker within an aged care facility.Interpersonal Skills: Excellent interpersonal skills with a high level of emotional intelligence, capable of building rapport and trust with stakeholders, including families and carers, while clearly articulating the role of social work.Time and Referral Management: Strong time management skills with the ability to balance and prioritise multiple responsibilities and referrals from various sources.The Candidates We Seek Will Have:A Bachelor's degree (minimum) and eligibility for membership with the AASW.A minimum of 5 years of clinical social work experience.A current National Police Clearance.Full COVID-19 and influenza vaccination status.Demonstrated ability to work both autonomously and within a team.Excellent communication and organisational skills to thrive in a fast-paced environment.What We Offer:A supportive employer who is a strong advocate for the role of Social Work in aged care.Access to clinical supervision from a senior Social Worker (monthly).Remuneration as per the Health Professionals and Support Services Award.Opportunities to explore other roles with our partner hospitals and aged care homes.
