By empowering families to take control of their wellbeing, the Link Worker helps clients connect with community resources that address social isolation, loneliness, and other related health and wellbeing concerns. This role involves building strong partnerships with local services and supporting clients through personalised care and ongoing coordination. The Social Prescribing Link Worker sits at the centre of a new trial program, dedicated to enhancing the understanding and capacity of social prescribing within community settings. As such, the role provides an exciting opportunity to contribute to program co-design and implementation, utilising your own skills to build the program from the ground up.POSITION RESPONSIBILITIESThe items listed below are indicative and not exhaustive.Client SupportConduct holistic assessments of clients' social, health, and wellbeing needs to understand their experience of social isolation and/or loneliness, any co-existing social, health, and wellbeing concerns and the complexity of need for the family as a whole.Conduct risk assessments and make referrals to specialist services as required for clients with complex needs that may be outside the scope for social prescribing or require specialist intervention.Establish trusting relationships with families, using behaviour change techniques to support lifestyle changes that improve their wellbeing.Co-develop tailored plans that leverage clients' strengths, connecting them with appropriate community groups, services, and activities. Provide support throughout their social prescription journey.Allocate brokerage resources to help individuals and families access social prescribing services, activities and support where appropriate.Maintain accurate case notes on all contact with clients.Referrals and PartnershipsEstablish partnerships with local services and organisations (e.g. health services, schools, employment services) to support referrals of clients who are experiencing social isolation and/or loneliness from these settings.Establish partnerships with local community groups to support referrals of clients to these settings to alleviate social isolation and/or loneliness.Community DevelopmentWork collaboratively with the neighbourhood centre team to map community assets and identify options for social prescriptions referral pathways.Build and maintain a database of local resources, activities, community groups and assets.Advocate for and support people to start new community groups where gaps are identified.Program Design & EvaluationContribute to the design and development of the Social Prescribing Link Worker trial approach plan.Maintain accurate records of client interactions.Participate in program evaluation to evaluate participant and program outcomes.Participate in a community of practice with other Social Prescribing Link Workers in the sector.Professional DevelopmentUndertake professional development, training, and external professional supervision relevant to the role and maintain appropriate professional qualifications.Organisational Contributions:All HUB personnel are expected to:Adhere to all relevant legal, professional, and organisational standards, including compliance with legislation, risk management guidelines, internal policies, and professional development requirements.Maintain accurate and comprehensive records of all work, ensuring data integrity and timely reporting to funding bodies as required.Actively engage in and contribute to a collaborative, supportive, and productive team environment through regular participation in professional development, staff meetings, operational planning, and organisational initiatives.Demonstrate a proactive approach to resource management, optimising the use of time, materials, and equipment to maintain operational efficiency.Uphold the highest standards of quality assurance in all work, promptly addressing any risk management issues and ensuring consistent delivery of services.Promote and ensure a safe and healthy work environment, adhering to safety guidelines and fostering a culture free from discrimination and harassment.Engage in self-care practices and manage workload to prevent burnout, supported by the organisation through access to an employee assistance program or external supervision as needed.Be adaptable and responsive to changing priorities, tasks, and organisational needs, undertaking additional duties as reasonably directed by the Neighbourhood Centre Manager or Director to support the ongoing success of the organisation.WORKING CONDITIONSPositive and supportive workplace with flexible and welcoming culture.Office based work primarily from HUB Community Projects office located in Inala, with regular required travel to various local locations. Some work from home is possible, subject to operational requirements and agreement.Travel is an essential part of the role for effectively delivering outreach Link Worker services and employees must use their own vehicle or have reliable transportation to access the various work locations.Some flexibility to adjust your work hours and accrue time off in lieu, accommodating both professional and personal commitments.Additional paid leave is typically offered over Christmas/New Year period.Involves working with people with complex social issues and possible exposure to sensitive or distressing situations.Access to external supervision and an Employee Assistance Program (EAP) is available.ACCOUNTABILITYReports to the Neighbourhood Centre Manager.Overall, accountable to the Director and Management Committee of HUB Community Projects Inc.SELECTION CRITERIAEssential Qualifications, Skills, & ExperienceA degree in social work, human services, or similar degree/qualification and/or significant experience in equivalent work.Possess a valid driver's license and have access to a comprehensively insured vehicle, OR demonstrated ability to travel efficiently by other means between varied work locations for community development activities and partnership building.Eligible to obtain or have a current Working with Children Check (Bluecard).First Aid, CPR and Mental Health First Aid or willingness to obtain.Desirable Qualifications, Experience, and AttributesKnowledge of local community networks and services, as well as understanding of social prescribing, motivational and behavioural change techniques with a demonstrated ability to problem-solve and overcome barriers to client engagement.Strong assessment and consultation skills with families and children, and experience in providing structured, empowering support to improve health and wellbeing outcomes - including a demonstrated understanding of working child safe manner.Experience in working in a culturally sensitive way with diverse communities, including Aboriginal and/or Torres Strait Islander peoples, and those from culturally and linguistically diverse (CALD) backgrounds.Ability to manage and prioritise workload and client demand in line with community needs, individual support requirements and within scope of a social prescribing model.Strong organisational and administrative skills, including experience managing and recording confidential information, taking comprehensive case notes and contributing to data collection and reporting processes.Strong interpersonal and communication skills, with the ability to confidently engage with a wide variety of stakeholders and build relationships with community service partners for the delivery of the program.Demonstrated experience contributing to program/project design, with a flexible and proactive approach to program/project delivery.HOW TO APPLYClosing date for applications is Friday November 22nd at 5:00pm.
