Job Description About the Role Effectively provide early intervention, crisis management and aftercare services to Clients referred to the Social Health Culture Care Connect program. The role will assess and support the needs of families and individuals by operating within a strengths and priority-based framework. The Social Health Care Coordinator will deliver case management and crisis support for clients, providing a range of assessed social and wellbeing supports to clients in meeting their holistic recovery needs. Supports will be goal driven and focussed on building the resiliency and capacity of individuals.
Duties Intake Referrals and Provide Quality Support Data Management Appropriately respond to critical incidents, providing wellbeing first aid to clients in distress. Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems. Provide integration and assisted referrals and coordination between ATSICHS services to facilitate the delivery of an efficient, responsive, and integrated mental health and social needs service to patients. Liaise with other community organisations and government agencies for the purposes of information gathering, referral, networking and promoting ATSICHS Brisbane Services. Annual salary $74,351 to $82,964 per annum (Depending on experience and Qualification) 11.5% Super + Not for Profit Salary Packaging (Increases your take home pay). Desired Skills and Experience Required Tertiary qualification in Social Work or Human Services or related field and/or demonstrated experience in providing support and case coordination to people with complex needs. Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals. Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management. Ability to work with diverse people and presentations in providing case managed social work driven supports. Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services. Demonstrated ability to record accurate and consistent case notes. Demonstrated IT capability - experience in computer-based data management systems and Microsoft Office. A willingness to travel across the ATSICHS Brisbane region of services and work across different sites. A willingness to provide social work supports and engagement to patients outside of a clinic environment. Working with Children's Card (Blue Card) and NDIS Worker Screening (Yellow Card) or be eligible to apply. Preferred Previous experience in a multi-disciplinary team environment.
