We are currently seeking a committed and passionate Service Coordinator to join our growing team in Tottenham, Victoria.
Position Overview:The Disability Service Coordinator will facilitate and manage the onboarding of new clients, create care plans for participants with disabilities, and link them with internal support services.
What We Provide:Free on-site parkingComplimentary drinks, breakfast, and lunch provided by our caféFun and easy-going work environment with friendly staffPaid annual leave for your birthdayPerformance recognition, Employee of the Quarter awardsCareer progression opportunitiesResponsibilities:Work respectfully with clients with varying support needs, their families, support networks, and the community.Ensure compliance with the NDIS code of conduct while providing support and care.Enroll new participants in chosen programs, including creating and issuing quotes and service agreements.Prepare sign-up packs and show bags.Design enjoyable and engaging school holiday events for people with disabilities and source facilitators to run programs.Assist in the development, implementation, and review of program planning to effectively engage people with disabilities.Arrange and conduct care team meetings, including taking meeting minutes.Ensure participants' medication, epilepsy, behavior support, or other related plans are up to date and communicated within care team meetings.Source and restock materials and consumables for classes.Oversee and supervise classes.Assist with support worker rostering.Arrange inductions, meet and greets between support workers and the participants they will be supporting.Collect support worker case notes, ensure they meet standards for each session, and identify incidents from case notes.Ensure incident reporting is completed and submitted as per procedures.Brief support workers on changes to participants' support needs and provide briefings upon arrival for allocated participants.Manage room/location setup to ensure safety and accessibility.Support evidence-based practice through high-level administrative skills, documenting outcomes for individual participants.Develop and implement continuous improvement initiatives.Actively contribute to the business in a positive manner.Perform all other duties as directed.Minimum Requirements:Previous experience in a similar role with a demonstrated understanding of the NDIS.Excellent verbal and written communication skills.Exceptional time management skills.Demonstrated competence in delivering services that ensure participants' safety.Ability to provide the highest level of customer service and response.Commitment to quality improvement and evidence-based practice.Proven ability to prioritise and work proactively, organizing and managing own workload.Strong problem-solving skills.Demonstrated reliability, punctuality, and trustworthiness.Strong commitment to a person-centered approach.Ability to work independently with disabled individuals to achieve social and self-care goals.Positive attitude with a nurturing and kind nature.NDIS Workers Screening Check.Australian Driver's License and use of own car.Current First Aid Certificate.Infection Control Training COVID-19 module.Workers Orientation Certificate.Working with Children's Check.Passion and drive to make an impact in the lives of individuals living with a permanent disability.Certificate III or higher qualification in Disability Services or a relevant qualification.NDIS industry experience.Experience in a similar position with demonstrated experience in researching and organizing quotes and report writing.Experience working in a day service.Experience with incident management and positive behavior support plans.Observant and attentive to act quickly to resolve issues, incidents, or conflicts.Schedule:Monday to Friday8-hour shiftLicence/Certification:NDIS Workers Screening Check (Required)Work Authorization: Australia (Required)Competitive salary ranging from $50,000.00 – $60,000.00 per year.Supportive and inclusive work environment.Opportunities for professional development and growth.This is your chance to make an important contribution to the lives of people with disabilities by adding value through contemporary and professional skills.
