As a member of our CO-ACT team, you'll work in a supportive, family-like environment where collaboration is key, and your input on improving systems and practices is highly valued.About the OpportunityWe are seeking an experienced Senior Speech Pathologist to join the Statewide Care of the Older Person and Community Transition (CO-ACT) program.
In this role, you will provide high-quality care to patients with complex swallowing, communication, and cognitive disorders, supporting improved functional outcomes and discharge planning.
You will collaborate with an interdisciplinary team, contributing to the development of Models of Care and patient pathways, while also working closely with external service providers to ensure smooth patient transitions.As the Senior Speech Pathologist, you will oversee the planning, coordination, and evaluation of speech pathology services, representing the profession on key committees.
You will report to the CO-ACT Allied Health Senior Manager and maintain professional links with the Director of Audiology and Speech Pathology.
Additionally, you will ensure compliance with SA Health requirements through external supervision, if needed, and foster relationships with community organisations to support patient care.About YouWe are looking for a skilled and dedicated professional with the following key attributes: Proven ability in conducting assessments, goal setting, and delivering evidence-based, consumer-centred care in speech pathology.Strong collaboration skills to work effectively within a multidisciplinary team, while fostering a culture of inclusion, critical thinking, and respectful communication.A minimum of three years' clinical experience in relevant settings, with expertise in managing dysphagia and cognitive/communication disorders.Excellent organisational, communication, and problem-solving abilities, with a commitment to maintaining high professional standards and ethical practice.
