Senior Operations Investigator

About the role
As part of the Operations Safety, Training and Resilience Division (OSTR), the Senior Operations Investigator plays a key role in applying Human Factors methodologies to lead and support safety investigations and risk reduction.
Key Accountabilities include:

Collaborating with the Operations Group to analyse complex human and organisational risks, ensuring investigations drive organisational learning and align with safety standards.
Leading the development of tools, processes, and root cause analysis techniques to enhance investigation capabilities and improve safety outcomes across V/Line.
Streamlining and refining the incident reporting process for V/Line Operations.
Creating easily adaptable tools and processes for frontline supervisory staff to achieve better investigation results.
Establishing a systematic approach for post-incident analysis, focusing on identifying root or systemic causes.
Monitoring incident reports and determining the appropriate level of investigation, whether conducted by Operations staff, other V/Line functions, or external resources.
Building investigation capacity within V/Line Operations by implementing a comprehensive training program for Operations functions.
Preparing investigation reports for the Executive and Board, alongside monthly safety updates.
Developing a forward-looking Occurrence Management improvement strategy, including an incident management framework and corrective action coordination.

Due to the nature of the role, availability for on-call duties outside regular business hours is required, including site visits, travel within Victoria or interstate, and overnight stays.
About you
With a proven background in Human Factors and incident investigation methodologies, you possess strong expertise in various investigation techniques and causation models. Additionally, you are skilled in delivering training and coaching to enhance investigation outcomes.
Our ideal candidate will have:

Extensive experience in railway operational incident investigation (essential).
Formal qualifications in incident investigation and Occupational Health and Safety (highly desirable).
Proficiency in conducting safety investigations, preferably within the rail industry, with expertise in post-incident interviews, data collection, and Human Factors analysis.
In-depth knowledge of Human Factors and incident investigation methodologies.
Expertise in identifying Human Factors contributors to safety incidents and conducting Human Factors activities/analyses to support safety improvement.
Understanding of Rail Resource Management tools (advantageous).
Certificate IV in Training & Assessment and formal training in causation models.
Membership with a professional Human Factors institution/association.
