Rehabilitation Assessor North West Private Hospital, Everton Park Permanent Part Time The Opportunity North West Private Hospital is opening an Inpatient Rehabilitation Ward in February 2025. As a Rehabilitation Assessor (RA), you will act as a clinical and professional resource person demonstrating broad knowledge and clinical expertise in rehabilitation programs.
Your duties will include (but are not limited to): Completing pre-assessments in conjunction with the rehabilitation multidisciplinary teamDetermining patient suitability and readiness for commencing rehabilitationFacilitating community and acute patients in the appropriate rehabilitation streamEnsuring patients and carers are fully informed of the available services and processesSupporting rehabilitation bed coordination and patient flowDemonstrating strong commitment to quality care, continuous improvement, and excellent communication to all stakeholders.Skills and Experience To be successful in this role you will demonstrate: AHPRA registration as a Registered Nurse or Allied Health professionalSuperior communication, negotiation, consultative and interpersonal skills, able to build strong business relationships at all levelsStrong teamwork and leadership skills, able to work autonomously whilst contributing to the teamAn understanding of private health rehabilitation admission criteriaBiopsychosocial/rehabilitation clinical reasoning and assessment skillsUnderstanding of Functional Independence Measure (FIM)Experience in complex case management and clinical decision makingA commitment to client-centered interventionsProficient computer processing skills and documentation skillsWhat's in it for you?
