Casual position - 2 days per week Incredibly rewarding community based role Salary Packaging benefits up to $15,900 annually to maximise your take home pay We are looking for dedicated and experienced Registered Nurses to join our Home Care Services Team supporting our clients across the Eastern suburbs of Adelaide.
About the role This role is responsible for delivering high quality clinical and care coordinator services to achieve person centred care for clients/care recipients that is responsive to their assessed needs.
Key responsibilities of this role include but are not limited to; Conducting client on-boardings, reviews, assessments and developing budgets for clients, resulting in clients having a current and holistic assessment and a service plan which manages all risks associated with their assessed needs, within their service budget and with a health promoting focus Escalations, purchasing, upgrades and reviews as well as liaising with key stakeholders including rostering, finance/admin teams and other home care or Southern Cross Care employees Delivering nursing services to the client in their home or via telehealth which is best practice and maximises their ability to continue to live in the community.
Ensuring effective management of clinical matters within the region Providing effective leadership to direct care staff in the region to meet client/care recipient needs in a timely and effective manner Ensuring all assessments and documentation meet legislative requirements Monitoring staff in the region ensuring the required levels of competency is achieved including conducting Home Care Assistant competency assessments Conducting orientation and training for staff within the region, under the direction of the Care Manager - Home Care Services For further details regarding the scope of the role, please refer to our website or the attached position description.

You'll work amongst a multi-disciplinary team to provide integrated care that promotes healthy ageing and well-being, social engagement, prevents frailty and avoidable decline and achieves reablement for clients to optimise their ability to live better in their own home.
The successful candidate will have the following skills and experience: Bachelor of Nursing with current AHPRA registration as an Registered Nurse Current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009) Current and unrestricted driver's licence & own registered and insured car Sound communication skills with a level of English literacy that enables you to speak in a manner that older people can understand, as well as the ability to read written instructions and to write clearly and concisely particularly in progress notes Exceptional time management skills to manage own workload to meet deadlines and to support other staff Outstanding problem solving skills to balance supporting resident's desire for independence and self determination to make their own choices against potential risks and duty of care High level understanding of the requirements to support residents living with dementia including those who display challenging behaviours Excellent knowledge of the Aged Care Standards particularly as they relate to accreditation You will be suited to this role if you have a positive mindset and can prioritise well Ability to demonstrate integrity, calmness and personal resilience Passionate about person-centred care, enabling clients to maintain maximum independence, you will be able to seek creative solutions in your approaches to overcome any barriers About us Southern Cross Care is one of Australia's leading charitable aged care, health and retirement living service providers.
