About the role As a Registered Nurse with BaptistCare at home, you will be responsible for coordinating the delivery of high-quality care and services that support our customers to remain living at home.
You will be responsible for the clinical care and governance across the NSW Central Coast region, from Woy Woy to Doyalson.
This is a Permanent full-time position working Monday to Friday.
You will be operating independently in the community and working from home, with support from our Morisset office and a state Clinical Team.
You'll develop genuine connections with our customers and their loved ones and assist them to implement innovative strategies that will optimise their health and well-being and support their independence.
A typical day can include:
Providing guidance and clinical support to Care Facilitators caring for HCP clientsWorking alongside our customers to set individual goals that are realistic and achievableImplementation of the latest best practice and evidence-based approachesThe position includes travel to clients and working from your own home-based office; we provide a mobile phone and laptop.About you You are a born communicator with high organisational skills committed to delivering exceptional customer service. You think outside the box and lead our customers to care solutions they trust. Your vision and enthusiasm lift those around you.
To be shortlisted, you will need:
