Key ResponsibilitiesUse a holistic approach to conduct client assessments and develop individualised care plansProvide client-centred care that promotes independence within the communityDevelop strong relationships and communicate effectively with clients, families, care providers and health professionals to establish goals, provide support and education, and ensure needs are being metMaintain accurate and detailed records of client progress and treatment plansWork with autonomy and flexibility, enhance your clinical skills and build meaningful relationships with clientsManage time efficiently and meet deadlinesAbout youYou have passion with purpose and are driven by excellence.

In addition, you will have the following skills:
