We are seeking a motivated Pharmacotherapy Area Base Network Coordinator to join our team!
About the Role The Pharmacotherapy Area Based Network (PABN) is an initiative of the Victorian Government. La Trobe Community Health Service has been funded as the lead agency, with Primary Care Connect (PCC) providing services across the Hume catchment area.
We are seeking a Pharmacotherapy Area Network Co-ordinator (PABN) in the Hume region to continue the work to develop this initiative in partnership with the Consortium members.
The successful applicant will develop close links with General Practitioners (GP's), pharmacists and other primary care workers and services within the Hume region while developing and ensuring effective and efficient systems and processes to support high quality service delivery. They will support the Rural Addiction Medicine Pharmacotherapy Specialist (RAMPS) service including education events for GP's and pharmacists to increase local knowledge in pharmacotherapy.
The PABN will contribute to the continuing development of the role and the pharmacotherapy network while working to integrate primary health care and the specialist AOD sector, including providing education and secondary consultation to the AOD team at Primary Care Connect.
Key Selection Criteria The following criteria must be met and addressed in applications for consideration for this position:
Tertiary Qualifications in Nursing, Pharmacy or Alcohol and Other Drugs A demonstrated understanding of the role of primary health care in particular General Practice and the barriers to engaging GP's and health care providers. Significant experience in leading, developing and implementing successful health care projects A demonstrated understanding of substance misuse and the principles of harm reduction. Sound knowledge of pharmacotherapy and its application within the Victorian AOD treatment sector. Proven ability to multi-task, set priorities and meet strict deadlines. Exceptional written and oral communication skills. Including proven skills in negotiating, developing and managing cooperative relationships across multiple and diverse stakeholders. Doctors and pharmacists and health care providers are key stakeholders to the program success. Well-developed computer skills, including email, Microsoft Office suite and Excel.
