You'll dive into diverse tasks, from crafting Medication Action Plans and conducting thorough medication reconciliations to providing discharge counselling and pharmaceutical reviews. Each interaction will be an opportunity to apply your clinical expertise, tackle complex decision-making, and contribute to continuous improvement initiatives. Collaborating closely with senior clinicians, you'll also help refine pharmacy practices and protocols, ensuring high-quality care for all patients.
