Job Purpose Registered Nurse
You will be carrying out a range of administrative duties alongside a service caseload. Ensuring individualised services are developed and delivered to customers. Managers are responsible for the development, maintenance and review of individualised support plans in consultation with customer, their family or guardians and the key health professionals, ensuring efficient and effective delivery of professional nursing care and support to customers, respecting customer's rights, and in consultation and cooperation with the customer, their representatives, their medical practitioner(s), and other health professionals. This role involves the providing of effective leadership, supervision, and clinical support to staff where necessary, ensuring care is provided in accordance with the relevant Professional and Industry Standards, and all legislative requirements.
Main Duties
• Provides and delivers individual and personalised support to clients in accordance with their care plans and/or medical practitioner's directions, responsibilities may include working under the guidance and supervision of medical practitioners, working with allied health professionals, monitoring and communicating the customer's condition, and other health and wellness related activities.
• Ensure that all documentation is completed in a timely and accurate manner.
• Attend / facilitate and participate in staff meetings and training sessions as required.
• Ensuring that all responsible steps are taken to protect personal safety at work and avoid adversely affecting the health or safety of any other person at the workplace in accordance with requirements under the Work Health and Safety legalisation.
• Develop and participate in the Performance Development Plan process. Participate in activities aimed at the continuous improvement of services

• Ensuring compliance with DCC's Staff Handbook, including; relevant policies and procedures. Principal Accountabilities
• Ensure support plans are developed in partnership with the customer and their representative/s, and support plans are monitored and reviewed on a regular and ongoing basis and in response to changes in customer needs.
• Ensure services are delivered are in line with the person centred and consumer directed principles.
• Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.
• Identify and participate in personal professional development for yourself and the office team.
• Ensure compliance with all DCC's policies and procedures, statutory and contractual requirements in relation to management of service delivery.
• Manage communication with key stakeholders, internally and externally, building strong relationships with brokers and external stakeholders.
• Apply and uphold principles of equality and anti- discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
• Perform nursing services in a manner that complies with all relevant legislative, common law, regulatory requirements, and professional standards affecting nursing practise.
