The Continuing Care Team model of care is a non-acute follow up to people with mental health issues, requiring specialist mental health treatment that is not met by generic services. This service is aimed at providing case management support and treatment for consumers with a severe/persistent mental illness aged 25 to 65 years. We are seeking a new team manager to join our team in Pakenham on a full time basis, working Monday to Friday 830-5pm. This position is open to Registered nurses, Occupational Therapist or Social Workers.A Day in the Life In this leadership role you will work with adult consumers with varying degrees of psychiatric or mental illness and/or drug and alcohol dependency problems. Continuing Care Team Manager is expected to provide both clinical and organisational oversight to the Continuing Care Team.
