This role will see you engage with those experiencing homelessness and provide case management to assist with overcoming barriers and successfully maintaining an independent tenancy: Key responsibilities: Undertake networking opportunities within the community to successfully engage with clients experiencing homelessness Coordinate services and provide supported referrals for clients for housing, health care, employment, financial and other relevant services Advocate on behalf of clients accessing other services Develop and implement support plans with clients Ensure effective and appropriate communication with staff within all programs in the delivery of the service via regular team meetings Ensure a diligent approach is taken to record keeping and the principals of confidentiality and privacy Provide regular communication with the Housing Services Coordinator and assisting with relevant data collection and reporting This position is full time, fixed term (12 months initially) based at our Maryborough office.
About you: You will demonstrate experience in a similar role, including experience in case management and/or provision of support services.
In addition, you will have relevant qualifications and/or experience in a human services framework for people presenting as homeless, including a demonstrated understanding of the needs of people experiencing homelessness or housing crisis and knowledge of relevant local support services.
You will also have: A talent for building relationships and working in partnership with a diverse range of stakeholders Excellent communication skills, including the ability to communicate clearly within complex and diverse conversational contexts A client centered approach, including provision of respectful, supportive, and confidential customer service Self-motivated with the ability to work independently as part of a wider team and adapt in a fast-paced environment The ability to achieve work objectives, even in difficult circumstances whilst remaining positive and calm A current driver license The ability to work independently as part of a wider team and adapt in a fast-paced environment A commitment to the right of all people to affordable and sustainable housing Please note that a satisfactory police check is required for all staff and those with client related responsibilities are required to provide a NDIS and Working with Children Check (WWCC).
