About the Role As an Outcomes Lead at Community Connections, you will play a pivotal role in mentoring and guiding a team of Disability Support Workers to deliver high-quality, person-centered outcomes for the people we support. Your responsibilities include:
Understanding, developing, and refining individual programs of support in line with a person's NDIS goalsDeveloping links to a person's community that offer them greater opportunity for inclusion and the development of valued social rolesOnboarding, supporting, supervising, and developing of assigned team membersManaging transport logistics during service deliveryCollaborating on service quality monitoring, planning, analysis, and reportingWorking with the Leadership team to ensure smooth rostering and system updatesCollaborating with peers to maintain accurate person-supported information and documentationReviewing and approving Carelink entries in line with payroll and billing requirementsManage and Follow Up on Incident ReportsDemonstrate incorporating practice leadership in daily operationsDelivering hands-on disability support work as requiredStudent placement supervision as requiredAbout You Be a natural influencer, modelling a positive attitude, professional conduct and unwavering commitment to quality outcomes and service delivery for staff, individuals with disability and the community.Possess a deep understanding of the NDIS, its funding, and systems.Have experience in leading and inspiring small teamsHave prior support worker experience including demonstrated understanding and knowledge of the tasks of a disability support workerBe committed to service excellence, human rights, and zero tolerance to violence, abuse, neglect, and exploitation.Have experience running a day service (desirable)Requirements Certificate IV in Disability desirable but not mandatoryA current NDIS Worker Screening and Working with Children Check (we can support you through the process of obtaining one or both checks if you don't already have them).Current unrestricted drivers' licenceCurrent CPR and First Aid certificationHigh level professional written and verbal communication / presentation skillsHigh level organisational and time management skills including a proven ability to prioritise, meet deadlines and work autonomouslyCompliance with OHS&W regulations, legislation, and Policies and Procedures is required. Successful completion of pre-employment safety screening checks, mandatory training, and their subsequent renewals is necessary.Why Join Melba?
