Your Role As an Occupational Therapist, you'll be looking after the most important people in our organisation - our clients.
This is a great opportunity to work across a wide range of client bases - from needing help with basic equipment, through to home modifications to assist our clients to stay safe in their homes and live as independently as possible Your tasks will include: Conducting in-home clinical assessments for physical and neurological conditions Developing and implementing individual intervention plans tailored to clients' specific needs and goals ADL and mobility assessments, as well as prescribing aids and equipment to enhance independent living skills e.g.
hoists, mobile commode, wheelchairs Environmental assessments and recommending home modifications to minimise fall risk and improve access Collaborating with the multi-disciplinary team, other health care professionals, community providers, and family members to provide holistic care Where appropriate, reporting to case managers regarding client conditions and occupational therapy interventions and recommendations.
Additionally, making referrals (nursing, physiotherapy, dietetics) as needed to ensure optimal client health outcomes Applying for equipment and mobility aids through MASS, GEAT2GO and NDIS funding programs Your Attributes We're looking for a person who has what it takes to make a difference in our clients' lives.
