About the opportunity:
Position: Nurse Navigator - First Nations Chronic Conditions (Identified)
* Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
Vacancy: Permanent part-time, 56 hours per fortnight
Location: Nurse Navigator Chronic Disease Service, Townsville University Hospital
This role focuses on supporting our Aboriginal and Torres Strait Islander people with chronic conditions and complex care needs, particularly those transitioning between The Townsville Hospital and their local health service. The opportunity is to assist and reform wide service delivery to improve health care outcomes for this patient cohort with a proactive approach to streamlining their healthcare journey and engaging primary health care services.

Generous remuneration package value up to $121,354 p.a. comprising:
* Salary range of $66.85 to $72.58 p.h. (NG7)
* Annual leave with 17.5% loading
* 12.75% employer contribution to superannuation.

Requirements:
* Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
* Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
* Appointment to this position requires proof of qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA) and possession of a current practising certificate.
* Possession of class C drivers' licence.
* This is a Vaccine Preventable Disease (VPD) risk role. Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis and Tuberculosis.
* Probation period may apply.
* Pre-employment checks will be conducted including criminal history clearance.
