Epiphany Care is seeking a highly enthusiastic and experienced Support Coordinator who would establish a positive collaborative relationship between our clients and their support network in their local communities, build skills, overcome barriers, and promote their success toward their stated goals.What we need from you:Business development and marketing our service in the community to link with new participants, service providers, local organisations, and government agencies.Coordinate and implement holistic services related to housing, education, and health, NDIS supports, and specialist services for individuals with a wide range of disabilities.Research and manage a range of support options to suit individual needs across multiple service providers within budget.Obtain quotes for services and organise service agreements.Progress reports and recommendations on appropriate support for future plans are communicated to the NDIA in a timely manner.Monitor monthly billing and track billable hours.Advocate and promote community inclusion and person-centred supports.Understanding NDIS guidelines and processes is essential to provide advice and consultation to participants and their families and ensure compliance and quality service standards are met.Assist NDIS participants in planning to prepare for their NDIS plan review.Excellent communication skills and the natural ability to build rapport easily and connect with others is essential.Current police check or willingness to undertake one and NDIS screener check clearance.Previous experience (minimum 2 years) as an NDIS Support Coordinator, Local Area Coordinator, Planner, or similar role is highly regarded.Relevant qualification and/or experience in social work, case management/support coordination/disability industry preferred.Current first-aid (with 12 months to run) and CPR qualifications (with 6 months to run) or willingness to complete before employment.Current Working with Children's Check.Competent in using computers and other mobile apps.Current driver's vehicle license and comprehensive car insurance.Available to work from the office in Mayfield.To apply:Please apply via Seek by uploading a cover letter and resume.
