Position Overview: Due to an increase in demand, we are searching for our next superstar Support Coordinator to join our dynamic team. This is an exciting opportunity for individuals passionate about making a difference in the lives of NDIS participants and their families.
Key Responsibilities: Provide high-quality support coordination services to participants. Develop and implement person-centred support plans. Collaborate with participants, families, and other stakeholders to ensure seamless service delivery. Assist participants in accessing necessary services and supports. Monitor and review participant progress and adjust support plans as needed. Qualifications: Preferred minimum of 2 years' experience in NDIS support coordination, supporting high-intensity NDIS participants with complex disability support needs and challenging behaviours. Experience in NDIS Support Coordination or a related field is preferred. Strong communication and interpersonal skills. Ability to work independently and as part of a team. A commitment to delivering high-quality, person-centered services. Must have computer literacy and proficiency in Microsoft Suite. Current driver's license & your own fully insured and safe vehicle. Willingness to obtain a valid NDIS Workers Screening Check. Preferred Qualifications in Community Services, Disability, or other health-related disciplines (highly regarded). Tasks & responsibilities: Provide Support Coordination, and/or Psychosocial Recovery Coaching to NDIS participants with diverse support needs. Utilise a person-centred approach to build capacity, choice and control for the participants. Must have excellent communication skills and supported decision-making skills. Develop and implement support plans with clients and other stakeholders and monitor their progress and outcomes. Identify and provide a wide range of solutions to complex challenges, barriers and risks that participants face, working collaboratively with stakeholders.
