The Navigator Program is an intensive, targeted education support service where Case Managers work in collaboration with schools, young people, families, and external stakeholders to find ways to achieve educational goals.
In 2023-24, 847 young people were supported to re-engage with their education through the Navigator program.

About the role: The primary goal of the position is to provide active holding assistance and conduct intake assessments for young people waiting to support to re-engage back into mainstream education.
You will be supporting referrals for young people (12 - 17yrs) who have disconnected from school and assisting their carers/guardians. You will work collaboratively within a multi-disciplinary team, comprising of a Team Leader, Senior Case Managers, Brief Interventions Worker, a Groups Facilitator and other relevant services, bringing expertise and valuable understanding in specialist aspects pertaining to current disengagement challenges.
Make your mark: Complete intake assessments which gather client information and begin to explore their strengths, interests and barriers to mainstream education Present Active Hold cases at the Navigator Allocations Meetings Develop and maintain a professional and respectful working relationship with young people, their families, schools and other referrers Provide appropriate referral information for other support services available. Maintain intake information and assessments as required for the delivery of the Navigator Service. Regularly record information on the department's data management system Work as part of a team and participate in team meetings, joint work opportunities and case evaluation activities. Make it work: Inspiring leadership supporting individual agency and autonomy. Salary starts at $97,377, Level 5 SCHCADS. Permanent. Part time. 30.4 hours per week, please pro rata the above salary. Flexible through the increments up to 5.3 Positions to commence December 2024/January 2025 by negotiation. Ongoing position. Site based at our office in Preston with outreach across the region.
