About the Role As a Clinical Pharmacist at St Vincent's Northside, you will provide hands-on clinical pharmacy services, supporting patient care and working closely with a multidisciplinary team. Deliver comprehensive clinical pharmacy services in a hospital setting.Conduct medication history interviews and reconciliation at transitions of care.Provide medication counselling and education to patients.Monitor medication outcomes and manage adverse drug reactions.Work collaboratively with doctors, nurses, and allied health professionals to provide the best patient care.Participate in training sessions for pharmacy, medical, and nursing staff.Support quality use of medicines through various audits and projects. About You Prior hospital pharmacy experience will be well regarded, but not a requirement. We strongly encourage Community Pharmacists looking to transition into the hospital space to apply for this position. Hands-on training will be provided.
To be considered for the role, we require that you are a:
Registered Pharmacist with current AHPRA registration.Minimum 1-year experience as a Pharmacist in an Australian-based Pharmacy.
