Anglicare Central Queensland Limited | Emerald $82,306 p/a +superannuation + salary sacrifice Full-time -Fixed-term position About therole: We arecurrently seeking a full-time Homelessness Case Manager to join ourteam in Emerald. They will provide case management support topeople who are homeless, at imminent risk of homelessness or housedbut at risk of homelessness. Case management is provided to peoplein their home, in temporary living situations, in supportedhousing, in a community setting or in a publicspace. Whatwill a typical day look like? Work closely withparticipants to develop, action and review case managementplans to meet their individual needs. Provide practicaland personal support, crisis budgeting, information, advice,and community referrals. Provide initial andongoing assessment of participant needs, including riskassessments and safety planning as needed. Provide supportthat is proactive, persistent, reliable, practical, andcomprehensive in a manner that is respectful of the dignity andprivacy of individuals. Provide services ina way that maximises the participant's capacity forindependent living. Advocate and assistparticipants to access housing and specialist services forexample, drug or alcohol, mental health, legal, domestic, andfamily violence. Support socialcontact with family and friends, and forming new socialnetworks as needed. Support access toeducation, volunteering, employment, and leisureactivities. Cooperate andcoordinate with other services and where appropriatecollaborate and integrate service delivery. Maximise the use ofavailable mainstream and community resources, including social,to ensure the sustainability of participantoutcomes. Participate in casemanagement opportunities to ensure holistic support isavailable to families and individuals. Assist participantsto address immediate basic needs in times ofcrisis. MandatoryRequirements: Qualificationscommencing from a Certificate IV in Social Services, CommunityServices or at least 3 years' experience in a similarcase management role and enrolment into a relevant minimumqualification of Certificate IV in Social Services, CommunityServices on contract.
