We provide comprehensive services designed to empower individuals and families, helping them find stable housing and regain their independence.Position Overview: We are seeking a dedicated and empathetic Homelessness Case Manager to join our team for a 4-week assignment.
The ideal candidate will have a passion for social justice and experience in working with vulnerable populations.
As a Homelessness Case Manager, you will provide personalized support to individuals and families experiencing homelessness, helping them navigate the complex systems of care, secure housing, and access necessary resources.Key Responsibilities:Client Support: Conduct intake assessments and develop individualized service plans for clients.
Provide one-on-one support and counseling to help clients achieve their goals.Resource Navigation: Assist clients in accessing community resources, including housing, healthcare, employment services, and financial assistance.Advocacy: Advocate on behalf of clients to ensure they receive necessary services and support.
Work with community partners and agencies to address barriers and challenges.Documentation: Maintain accurate and up-to-date case records, including progress notes, service plans, and outcomes.
Ensure compliance with organizational policies and funding requirements.Crisis Intervention: Provide immediate support and intervention in crisis situations.
Develop and implement safety plans as needed.Program Development: Contribute to the development and improvement of programs and services based on client needs and feedback.Qualifications:Education: Bachelor's degree in Social Work, Psychology, Human Services, or a related field.Experience: Minimum of 2 years of experience working with individuals experiencing homelessness or similar vulnerable populations.Skills: Strong interpersonal and communication skills.
