The Home Care Package Co-ordinator performs an integral role in assisting in individualised services being delivered to our customers in their homes, ensuring our customer can remain living their optimal lives, safely at home for as long as possible.
Along with this, the Home Care Package Co-ordinator will also connect with and form relationships with key community stakeholders in the region.

Duties and responsibilities may include: Working with or under the guidance of medical practitioners and Allied Health professionalsEnsuring care plans are monitored and reviewed on a regular and ongoing basis and in response to changes in customer needsEnsuring services are delivered in line with the person-centred and consumer-directed principlesMonitoring and communicating on the customer's wellbeing and reporting on progress of customersWorking with customers to help them participate in the daily activities they want to do, including hobbies, their interests, and moving about their home easilyEnsuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures, legislative requirements, and program-specific guidelinesAssist in customer service schedules including service planning, confirmation, and short notice placements or cancellationsLiaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal, and other regulatory bodiesAssist customers to access other alternative services where the services are no longer meeting their needsUndertake administrative duties including data entry, processing invoices, following up client enquiries, ensuring date noting of all interactions ensuring compliance, etc.Ensuring evidence is gathered and uploaded including quotes, confirmation of delivery of services, etc.Liaise with relevant community-based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectivesWork with customers with their budgets and monitor customers' budgets in accordance with program guidelinesCommunity Engagement: engagement with key community stakeholders, promoting Just Better CareIdentify and participate in personal professional developmentEnsure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service deliveryCommunication with key stakeholders, internally and externallyApply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety, and Environment requirementsConducting skills assessments of Community Support Professionals performing complex care services as requiredDemonstrating a commitment to best practices and evidence of continued professional developmentParticipating in staff meetings and attending training opportunities when requiredAny other duties as required to meet the ongoing needs of the organisationWhat you will need: Strong communication skillsStrong administrative, time management, and organisational skillsDemonstrated ability to learn CRM systems and other computer platformsCurrent CPR / First Aid CertificateCurrent and satisfactory Federal Police Check, NDIS Workers Screening Card & Blue CardCurrent Australian Driver's LicenceReliable, registered and insured vehicle (comprehensive insurance is preferred)Desirable: Cert III or IV in Individual SupportPrevious experience in the community care or health/aged care sectorNetworking experience and skillsNext steps:If you would like to join our growing team and become part of our friendly, professional network please Apply today.
