The purpose of the Identified Nurse Navigator role is to coordinate and improve patient-centred care for Australian First Nations Peoples, ensuring they receive culturally safe and high-quality care that supports their health priorities. By fostering partnerships, improving patient outcomes, and contributing to system improvements, the role enhances the patient journey, strengthens connections to community, and builds trust in the healthcare system.

About the Role Demonstrate advanced application of clinical and professional leadership skill and specialist knowledge to achieve integrated patient care and reduce fragmentation in the patient journey.Adhere to, develop and evaluate established guidelines, protocols, procedures, standards and systems of work as set by the organisation.Provide educational leadership and expertise, supporting a learning environment by mentoring and promoting team development opportunities to enhance and assure Person Centred Care.Provide a central point of communication, engagement and coordination for key partners involved in the individual patients care across primary, secondary and tertiary care providers utilising multi-modal communication pathways facilitating the patient focused care journey.This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community.
