Support Coordination is essential for empowering individuals with disabilities and facilitating the necessary resources to enhance a high quality of life.
For Love 2 Live Disability, it is essential that the successful candidate has the following knowledge and experience:
You must have comprehensive experience in a previous Support Coordination role and be able to demonstrate knowledge and understanding about the NDIS and how to assist in the implementation of NDIS plans in line with the NDIS legislation.Have a positive relationship with local providers and mainstream health services, to assist participants in establishing and accessing services which meet their needs to build their capacity for a higher quality of life.Have a forward approach in collaboration to ensure participants, their family, and stakeholders feel valued and well supported.Have great attention to detail with documentation and report writing as Support Coordinators are valuable in providing the NDIS with key information required to ensure adequate NDIS plans are built for participants' needs.Have great advocacy skills!
Although we aren't an advocacy service, as a Support Coordinator, you will have instances where you will need to advocate for the best outcome for the scenario, to ensure your participants can remain well supported, whilst abiding by the NDIS legislations and code of conduct.#J-18808-Ljbffr
