About your opportunity: In this position you will:
Implement individual assessment, identification and management of co-morbid chronic illness, mental health conditions, musculoskeletal or neurological impairment which might impact exercise physiology service provision, treatment, follow-up, multidisciplinary consultation, liaison with referring agents, discharge planning and evaluation of care using appropriate outcome and data measurement systems. Deliver Exercise Physiology services and health promotion education initiatives in collaboration with the Adult Acute Mental Health Inpatient Team to individual clients across a variety of health care settings. Collaborate effectively with clients, carers, families, colleagues, service providers and organisations using high level communication, negotiation and conflict management skills to optimise the quality of client care. Accurately and concisely document care and precisely record data on service activity and outcomes in accordance with legal, service and professional requirements.
