About UsApunipima Cape York Health Council is a community controlled Aboriginal and Torres Strait Islander health service, which provides culturally appropriate, family centred, primary health care services to the communities and people of Cape York.
About the role The Elder Care Support Coordinator is responsible for supporting clients to navigate access to appropriate local aged care support services and provide advocacy support to ensure clients receive a culturally safe assessment and plan development.
The position is fulltime, fixed term with an end date of 30 June 2025.
This position is identified where, under section 25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community. About You To be successful in this role you will have:
A minimum of three (3) years' experience as a social and community services worker, preferably in a not-for-profit organisation.Experience working with older Aboriginal and Torres Strait Islander people.Experience working remotely and independently within a multi-disciplinary team environment.Understanding of how to assist and support clients to access services, reduce service complexity and confusion.Ability to prioritise, meet deadlines, deal with matters of a sensitive and confidential nature, use initiative and solve problems of a complex nature.Sound understanding of My Aged Care and operational guidelines or the ability to quickly acquire knowledge and skills relevant to the Aged Care sector.Ability to maintain timely and accurate records, collect and analyse data and provide informative reports.Well-developed written and interpersonal communication skills including the ability to liaise with service providers.Understanding of the historical and contemporary issues that impact on Aboriginal and/or Torres Strait Islander people and their communities, particularly in Cape York.Ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people in accordance with community protocols and customs.Demonstrated understanding of the principles of Aboriginal and/or Torres Strait Islander community control.Must be willing and have the ability to travel in Cape York either by light aircraft or 4WD as required.Mandatory Requirements: Diploma in Community Services or Diploma of Community Development.Australian Federal Police Check or willing to obtain one prior to commencement.Current Queensland Drivers Licence.Current Blue Card positive notice or exemption, or willing and able to obtain prior to commencement.Qualification in Aged Care.Demonstrated experience in program planning, development, evaluation and implementation with the ability to maintain positive stakeholder and community relationships.Experience in delivering culturally appropriate programs such as community support, Domestic and Family Violence, Mental Health, Alcohol and Other Drugs.Experience in the community services field with strong community connections and networks across aged care providers, mainstream supports and community sector.What We Offer: Competitive salary range $106,108 - $114,171 p.a.
