– RN2CTemporary Full-Time, up to 12 months About the Role:Embark on a rewarding career path as a Dementia Support Clinical Nurse, where you'll have the opportunity to positively impact the lives of individuals living with dementia and their families.As a Dementia Support Clinical Nurse, you'll be responsible for collaborating with a multidisciplinary team to develop and implement specific dementia care pathways to enhance the care of individuals and their families.
You will also lead dementia pathway education throughout the wards to facilitate a common understanding across the multidisciplinary teams to assist in the progression of care for this patient cohort.
These areas of focus will support a more streamlined approach to the care of patients living with dementia who require additional supports to promote a positive quality of life.
To excel in this position, you'll need a strong clinical background, exceptional communication skills, able to work independently and achieve deadlines, and a genuine passion for improving the lives of those affected by dementia.
