As part of the Therapeutic Model of support, the role will use active engagement skills to develop rapport with clients whilst they are in crisis accommodation, with a view to having an established relationship once the client moves into the community. The Continuing Connections Case Manager will need to develop good relationships with community services to identify supports for clients in the community and assist with engagement. This role will work to provide clients with the support and community linkages appropriate to the client's needs, with a view to maintaining long-term accommodation.
This is a permanent part-time opportunity for 34.2 hours a week.
What you'll be doing: Support clients living in stable housing when exiting from crisis supported accommodation.Continue working towards and updating case plan goals as identified by clients.Engage with clients whilst in crisis supported accommodation to build rapport and provide a smooth transition into the community.Engage supports and stakeholders when client presents with or identifies issues requiring more support, with a view of sustaining stable housing.Have regular contact with clients exiting crisis supported accommodation over the extended period of support.Develop existing or new relationships and/or partnerships that will enable coordinated service delivery and improve service and client outcomes.Embed effective collaboration and communication with internal and external stakeholders.Comply with all record keeping, reporting and evaluation processes as applicable and directed.Work collaboratively with external agencies to develop a coordinated model of support specific to addressing the needs of each individual within this cohort.What we're looking for (key selection criteria): Have a relevant tertiary qualification in human services, and direct experience in the delivery of services to clients with complex needs.Demonstrated understanding of housing and homelessness policy, context and systems including strong understanding of tenancy support and the Residential Tenancies Act.Demonstrated ability to work collaboratively with others towards effective client solutions.Good interpersonal and communication skills, both verbal and written.Well-developed organisational and time-management skills.Demonstrated ability to build positive relationships and communicate with people from diverse backgrounds and abilities.Excellent problem-solving skills.Valid Victorian Drivers Licence (Mandatory).Demonstrated commitment to the values of Launch Housing and our mission to end homelessness.Salary: $44.37 - $47.74 an hour + Superannuation + Additional wellbeing leave + NFP Salary Packaging Benefits
