Our workplace is fast paced, challenging and supportive, where every employee plays a critical role in our success. The Continuing Care Team at the Mackay Community Health Centre is seeking applications for a Clinical Nurse to join their team on a permanent basis.
Your opportunity Provide leadership and support to DMHAODS Clinicians, Students, General Practitioners and other Primary Health Network Stakeholders to ensure that care is integrated across the entire patient journey to promote the wellness of DMHAODS consumers.Works with the Mackay HHS GPLO to strengthen connections and referral pathways to primary health care for Mental Health Consumers and promotes community based healthy lifestyle programs.Co-ordinate and oversee (within the program guidelines) the physical health assessment, monitoring and treatment of consumers receiving medications prescribed and administered within the DMAODS.Participate in General Wellness Clinic Activities autonomously, including provision of intramuscular injections to consumers of MHAODS where they are unable to access this in the private sector.Work in collaboration to manage the Clozapine Clinic, including assuming the role of Clozapine Coordinator as required.Aim to minimise relapse and improve consumers' ongoing mental and physical care while promoting a healthy lifestyle.Ensure all clinical monitoring for Clozapine is carried out in the appropriate time frames; educate and support Clozapine prescribing Doctors (within the Mackay HHS, including rural sites and GPs prescribers) and provide direct service provision to consumers and their carers.Provide support across the DMHAODS when required.About the Team The Wellness Clinic is located in the Community Mental Health Service.
This clinic operates to support the medication management and physical health monitoring of consumers under the care of Mental Health Services.
This clinic is run by a Clinical Nurse to promote recovery focused evidence-based nursing care and coordination of services for complex consumers.
