About your opportunity: Practice collaboratively and provide clinical leadership within the interdisciplinary health care team to promote the safety, security and personal integrity of individuals and groups.Effectively communicate at all levels across the organisation and with relevant stakeholders to promote effective and efficient referral process and team functioning.Assist families to negotiate health services and to resolve barriers in accessing referral pathways.Provide leadership to effectively plan, manage and evaluate referral processes at an advanced practitioner level within the primary health care area.
