An opportunity has arisen at the Newcastle Residential Facility for an energetic self-starter who is a highly motivated and experienced individual to join the site as a Clinical Care Coordinator (CCC) on a Full-Time basis. The successful candidate will be someone who is resident-focused and committed to quality care with strong people skills and a keen interest in delivering results.

What you will do Review progress notes daily and ensure that all appropriate interventions are actioned.Assess resident's clinical and psycho-social needs; develop care plans and intervention strategies.Excellent skills in assessing, documenting, and implementing resident's care planning objectives.Sound knowledge of the Accreditation Process.Sound knowledge of Aged Care Funding processes.Ability to work in a team setting, both as a participant and a leader.Well-developed problem solving, interpersonal, written, and communication skills.Assist Manager Residential Care or deputy with the admission process.What we look for Current AHPRA Registration.Bachelors/Masters Degree in Nursing.A Current Police Check or willingness to obtain one.At least 3+ years experience working as a Registered Nurse in Aged Care.Ability to assess training needs, design sessions, deliver staff training, and organise external education/training.Why work with us?
