About the roleThis is a Permanent Part-time position of 30.4 hours per week for a contract period of 7 months. The Client Liaison Officer undertakes assessment, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client in achieving their identified goals. This role supports clients to manage and monitor their budget to ensure care packages are financially sustainable and monitors the services provided to ensure these are following relevant service models and guidelines.
Skills and Experience Proven understanding of assessments, packaged care and case managementAbility to engage clients and work effectively with them to achieve identified goalsSupport and guidance to clients with budget management and financial sustainabilityExperience in monitoring and assessing services provided to ensure they are following service models and guidelinesExceptional communication skills – both written and verbal – and the ability to relate to a wide range of peopleAbility to advocate strongly for your clients and the services they requireA sound understanding of the community care system and the people who utilise this serviceDiplomacy and negotiation skills; people management capabilitiesAlong with a Certificate IV in Health, Community Services, Aged Care or Disability Support, current Driver Licence and a National Police Certificate. We also highly value qualifications relating to Case Management.
