Description: The Opportunity
As a Case Manager, you will play a pivotal role in managing and overseeing key NDIS services, including SIL, STA, MTA, ILO, and daily living supports. In addition to managing these services, you will be responsible for driving the development of new community access initiatives and innovative pilot programs to enrich the lives of our clients.
Key Responsibilities: Oversee and manage the delivery of SIL, STA, MTA, ILO, and daily living supports for young people.Join the business development team to develop and implement community access programs and pilot initiatives.Mentor and support the service coordinator focused on community access and new programs, providing guidance on NDIS processes and client support strategies.Liaise with clients, their families, and other key stakeholders to ensure services meet client needs and NDIS requirements.Collaborate with internal teams to maintain high service standards, ensure compliance, and track client outcomes. Skills and Experiences: What you bring Entrepreneurial, energetic spirit with a strong sense of alignment to our values and mission.At least 3 years of NDIS experience in support coordination, SIL or a Home and Living type of service. Formal qualification in social work an advantage.Knowledge of the NDIS sector, particularly SIL, SDA & ILO.At least intermediate IT skills with demonstrated ability to confidently generalize across different platforms.A passion for building strong relationships and being influential in your approach.
