$40.16 per hour Full Time Your Role
You'll be responsible for planning, managing and leading client care and support across a range of programs. You'll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care.
As a Care Manager you will: Manage and coordinate care services to ensure the highest standards of care Develop and implement care plans tailored to individual client needs Liaise with clients, families, and healthcare professionals to ensure comprehensive care Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals Ensure compliance with all relevant legislation and organizational policies Monitor and evaluate care services to continuously improve quality and efficiency Monitor the budget of Home Care Packages and ensure clients understand their funding and fees How You'll Benefit You'll become part of the Ozcare family, and in addition to the training and support that you would expect, you will also receive:
5 weeks annual leave as standard Annual leave loading Optional salary packaging, meal entertainment cards, and novated leases Private health insurance discounts Uniform and laundry allowance Employee Assistance Program For Success you need to: Demonstrate experience in case management and client care coordination in a Community Aged Care setting Have demonstrated strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough) Hold a current Police Screening Certificate Provide a copy of your current First Aid and CPR Certificate Hold a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check) Hold a current Queensland driver's licence Completed tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services) Welcoming All We welcome everyone to become part of the Ozcare family regardless of belief, ethnic or social background, health and gender.
