The Care Coordinator will be responsible for: Building strong connections with clients and their familiesOrganising support to clients with disabilities and monitoring and reporting on their progressAssessing client needs and resources for health, welfare, and other facilities and servicesResponding promptly to emails, enquiries, and requests from clients, staff, and managementMaintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding FrameworkReorganising work priorities to meet the needs of a fast-paced environmentSupporting all clients to meet their goalsConducting Client Care Assessments and Care PlanningSupervising and managing Care StaffParticipating on rotation with providing 'On Call' supportHaving experience in assessing and planning care needsSound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding FrameworkAll candidates must have the right to work in AustraliaOrganisational, administrative, and time management skillsDriver's LicenceAbility to Obtain: National Police CheckWorking with Children CheckNDIS Worker Screening CheckFirst Aid & CPR CertificateMust promote the values of the company and be prepared to plan supports using a contemporary approach.
