Responsibilities: Build strong relationships with clients and care workers, ensuring seamless communication and exceptional service.Manage a caseload of up to 35 clients living with various disabilities, coordinating their care plans, budget management, and promoting independence.Conduct home visits to introduce clients to our compassionate care workers and provide ongoing support.Utilise computer skills for efficient record-keeping and communication.Why Choose Right at Home Brisbane North And South:
