Care Coordinator

Our client is seeking passionate Case Coordinators to support their SA Community Care and My Home Hospital programs, which deliver care to patients in their homes.

Key Responsibilities: Utilise clinical skills to ensure comprehensive, client-centered management plans.
Conduct initial phone assessments, care planning, and goal setting for clients.
Coordinate in-home services to support clients' rehabilitation and recovery.
Build and maintain effective relationships with various stakeholders, including primary and secondary care organizations and a non-clinical support team.
Monitor patient progress and update care plans as needed.
Liaise with healthcare professionals and specialists.
Manage discharge planning, including referrals to long-term services.
Provide telephone support, education, and advice to clients.
Roster and Work Environment: Case Coordinators work on a rotating 7-day roster between 7am and 10pm, including alternate weekends.
The role offers the flexibility of working from home, with an on-call component.
Both part-time (0.6 FTE, 22.5 hours per week) and full-time (1.0 FTE, 37.5 hours per week) positions are available.
Part-time staff must be able to work three consecutive days.
Requirements: A minimum of 2 years of experience as a Registered Nurse with unrestricted AHPRA registration.
Recent experience in care coordination is desirable.
Experience in a hospital setting is desirable.
Strong customer service orientation and relationship management skills.
Excellent clinical reasoning and the ability to thrive in a fast-paced environment.
Proficiency in computer systems for accurate data entry into patient records.
